Record each stage
Use a request ID to connect the proposed action, policy decision, approval and downstream result. Include the agent, delegated principal, action, resource, rule version and any approval reference. Use an operation ID to correlate retries or reconciliation attempts.
Preserve a separate execution status. An allowed request can fail, time out or be withheld by a later check. A denied request should have no downstream execution result. Keep an unknown outcome open until you can reconcile it.
Limit what goes into the trace
Exclude tokens and unnecessary customer data. Capture just enough of the resource and arguments to explain the policy decision. Restrict access to records and define retention based on the investigation and operational needs of the system.
Run a deployment review
Check an allowed read, a denied request, a valid approval and an approval that has expired or changed. Attempt a direct tool call from the agent environment and simulate the policy service being unavailable. For each case, inspect the actual downstream outcome and reconstruct how it was authorised.
Start with a narrow set of actions and credentials. Expand the scope after the tests show that a legitimate action succeeds and blocked paths stay closed.
